Backport #5318: backport #3422 into Ruby 1.9.2

I have the same problem with Module inclusion described in #5143 with ruby_1_9_2

r32917 in ruby_1_9_2 on Aug 10 merged r32283 from trunk but this adds the bugs (#5143 and #3422) to the ruby_1_9_2 branch.

These bugs are fixed in r33163 on trunk.

These bugs do not affect ruby 1.9.2-p290 but do prevent me from using ruby_1_9_2 >= r32917
