Errors in Fiber/Continuation Documentation

Description

The first example in the Continuation documentation is no longer syntactically valid. In addition, the documentation for some Fiber/Continuation methods doesn't specify that 'fiber'/'continuation' needs to be required before they are used. The attached fixes these problems. (At least, it should; my RDoc installation is broken yet again so I can't check).
